I am ENROLLING my child, and I agree to the $250 new student enrollment fee. Enrollment is "perpetual'' therefore students are enrolled at our Preschool until they graduate or the parent(s) or school dis-enrolls the student. I understand that the standards of the school do not tolerate profanity, obscenity in word or action, dishonor to God or the Bible, or disrespect to the personnel of the school.

Realizing that my attitude toward the teachers and the policies affects the emotional and academic stability of ITIY child, I will support, pray for, and uphold the ideals of the school in every way and will abide by the discipline, regulations, and decisions of the administration. I further agree to uphold the policies of the school as stated in the
Student/Parent Handbook. I will at no time participate in destructive criticism of the staff or school, to my child or others, but will instead, if a problem arises, go directly to the teacher or administrator in a Christian manner as indicated in Matthew 18:15.

I herein authorize the school to employ such discipline as it deems wise and expedient for my child, including suspension and expulsion (note: these situations seldom occur.) I agree to cooperate with the school, to conduct myself and my home in such a manner that the Christian influences of the school will be upheld continually before my child.

I understand that the continued enrollment of my child in this school is dependent on my cooperation and support of the school, its staff, and its policies as outlined in the Student/Parent Handbook.
I absolve the school from liability to me or my child because of an injury at school or during a school activity.

1. The Preschool is generally for children two to five years old and is provided as a service to parents to prepare them for entering kindergarten when they reach enrollment age. Exceptions to the age requirement depend on evaluation by the Preschool Director with the approval of the School Board of Education.

2. Students in Trinity Lutheran Christian Preschool may be enrolled on a part-time basis . Changes in the enrollment schedule must be made in writing on forms available from the school office and approved by the Preschool Director. These forms must be signed by the parent/guardian and Preschool Director, and turned into the office at leas t two weeks before the change is to take effect. A charge of $25.00 is made for the enrollment change.

3. Enrollment in Trinity Lutheran Christian Preschool is "perpetual" and will continue until the student graduates from the program or the parent or school disenrolls the student. There is no prorating of months, with the exception of the vacation credit explained below.

4. Vacation Credit: Vacation credit is available for full time students ONLY. With 72- hour notice, you may have (2) weeks of vacation time each calendar year after being enrolled for a full 12 months.

5. Early withdrawals in Preschool due to unexpected circumstances will be charged a minimum of 2 weeks tuition after 2 weeks written notice is received by the office. Registration and first month's tuition are non-refundable.

6. If the Preschool must close due to circumstances beyond the school's control, such as a COVID-19 requirement, there will be no prorating of tuition, unless the school must be closed for more than 10 school days.

7. A $5.00 per hour charge will be added to tuition for extended care beyond the scheduled agreement as established on the Family Billing Form.

8. Statements will be sent via Brightwheel on the 1s t day of the month for the amounts due by the 10th of the month. Accounts pas t due on the 10th will be charged a $25.00 late fee. Tuition for the current month is due no later than the 10th of the month. We encourage the us e of the Brightwheel app to schedule auto payments or make one time payments. Payments returned for any reason will be billed a $25.00 return fee charge.

9. Repetitive delinquency in payment of tuition and fees will result in expulsion of the student(s). Parents or guardians responsible for enrollment who sign the application and pay the required registration fees will be charged all legal fees necessary to obtain payment of unpaid tuition and other costs as charged for the students enrolled.

10. "Active Church Members" are defined in the "Tuition Policy for Church Members". Copy of the policy is available upon request

COVID-19 guidance is continually evolving. Below are the protocols in place based on guidelines from Child Care Licensing (PIN 20-06-CCP) as of July 10, 2020. For more information, you can review the CDC guidelines. We will communicate with you if any changes to the guidance or recommendations are made.

Arrival
• One family member will escort child(ren) to the No Touch Temperature check station (If possible, the same parent/caregiver should drop off and pick up the child every day)
• Face covering is strongly recommended for sign in (see current CDC recommendations)
• Parents will sanitize prior to signing in

No Touch Temperature Check Station/Sign In 7:00 am - 8:30 am

• The No Touch Temperature Check In Station is located at the TLCS gate
• 6' Ground markers will provide family distancing
• The temperature table will be located at the gate
• No Touch thermometers will be used
• Ill children will not be permitted in the center (temperature 100.4 ° or higher)
• Children should be medication free/fever free for the past 24 hours
• Parents will sign in via brightwheel on iPad (sanitized between each use) and proceed to the 6' marker where they will transfer their child(ren) into our care.
• Parents will remain outside the campus at all times

NOTE: Entry After 8:30 am - Please enter through the school office Staff Assisted Entrance
• A Trinity staff member will guide the child at entry
• Children will place their belongings in a personalized cubby in an issued location
• Children will then be directed to the staff monitored restroom to wash their hands
• Limit of two children in the restroom at one time
• Children will then be directed to the designated morning area

Cloth Face Coverings: In California, the governor's requirement for masks exempts schools and child care. Due to this we are allowing the parents to make their own choice regarding their own child.

• Face coverings are meant to protect other people in case the wearer is unknowingly infected but does not have symptoms. Face coverings are not surgical masks, respirators, or other medical personal protective equipment.
• Face coverings will be worn by staff when social distancing isn't feasible.
• Face coverings are challenging for younger students to wear in all-day settings such as school; therefore, students will wear cloth face coverings as feasible during times when physical distancing is difficult. When wearing a covering, students will be frequently reminded not to touch the face covering and to wash their hands.
*Note: Cloth face coverings should not be placed on:

Children younger than 2 years old
Anyone who has trouble breathing or is unconscious
Anyone who is incapacitated or otherwise unable to remove the cloth face covering without assistance
In order to prevent trouble breathing, students will not be required to wear face coverings while outside engaged in active play or while outside and physical distancing is maintained.

Facility
We will utilize the Trinity rooms, the Fellowship Hall, outside field, blacktops and
equipment. All areas will follow physical distancing guidelines as appropriate.

Activities
Children will be with their friends enjoying all the wonderful activities and lessons we have planned for them, just as they would any other year. The only difference is that we will be monitoring the spacing of activities for the children. We will enjoy art, story time, science experiments, music, dance parties, sports, games, special event days, water play, free time with friends, picnic lunch on the grass,
and so much more.

• Children participate by age in all organized activities
• There will be a 10-1 teacher ratio or fewer per activity
• All activities are designed for and will promote 6' distancing

Supplies: Items to bring from home- Please label everything!

• A change of clothes and shoes left in your child's cubby
• A lunch with an ice pack and reusable water bottle
• Sunscreen
• Urban Infant Nap Mat if child will be napping for the day (may purchase on Amazon)
• Diapers and wipes as needed

Lunch & Snack
• Each day children will need to bring a lunch with 1 or 2 snacks individually packed for morning and afternoon.
• Children will eat at the lunch tables
• 6' Separation will be observed
• Lunch and snack sharing is not permitted

Restrooms Use and Hand Washing
• Individual use and or no more than two children in the restroom at any time
• Hand wash check
• Hand washing after activities and throughout the day

Close Campus
• Gates are locked
• 7am-8:30 am Trinity staff are supervising the gate, assisting check-in
• Parents will remain outside the gate at all times
• When the preschool office is not open, please ring the bell at the TLCS gate for assistance.

Illness
• Should a child become ill during the course of the day, children will be taken to a designated room away from the center
• Comfort and care will be provided while awaiting parent pick up
• Child pick up should be completed within an hour of telephone contact from office

Signing Children OUT of the center
• Staff assisted pick up of children
• One family member per pick up
• Observe the 6' distancing markers while waiting to sign out
• Staff will call child(ren) to leave the center
• Observe 6'distancing upon waiting for child (ren)
• Face covering is strongly recommended for sign out (see current CDC recommendations)

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited or limited the congregation of groups of people.

Trinity Lutheran Church and Preschool has put in place preventative measures to reduce the spread of COVID-19; however, the Church cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending the Church or Church-related activities could increase your risk and your child(ren)'s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Church or Church-related activities and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Church or Church-related activities may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Church employees, volunteers, and Church.!related activity participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at the Church or participation in Church-related activities {"Claims"). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Church, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Church, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Church-related activity.

I understand and agree that the law of the State of California will apply to this Waiver of Liability. I HAVE CAREFULLY READ AND FULLY UNDERSTAND All PROVISIONS OF THIS WAIVER OF LIABILITY, AND FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY AS DESCRIBED ABOVE:

Photo release: I hereby authorize and give full consent to Trinity Lutheran Christian School to publish in Trinity materials the photographs and video which my child appears. I further agree that Trinity may transfer or use these photographs in newsletters, advertising, posters, slide shows, videotapes and the like publications, and online.

I give my permission for the Staff of Trinity Christian Preschool to apply a sunscreen product of SPF15 or higher to my child when he/she will be engaging in outdoor activities. I understand that sunscreen may be applied to exposed skin, including but not limited to the face, top of ears, nose, bare shoulders, arms and legs.
